The tension after Lex realized that Clark is Superman was great and really heart-wrenching for a moment. I can't really see how anyone who's known Clark in Smallville would not immediately know that he's Superman, unless mind-wipes or serious denial was involved, but I love this kind of misunderstanding in fic, so it doesn't really matter.
